I think we will have to disagree on this. I do not believe that well-off women in the U.S. face significantly higher risks maternal or infant mortality because of the cost of health insurance or the quality of care they can access, except for some state restrictions on access to abortions and even those can be circumvented with money. I think that if you asked to international community, they would agree that the metric is narrow but useful.
