The problem apply not only government but also the private sector. Getting proper treatment regarding private insurance claims is hard work and rarely is proper compensation provided. After much time and effort applicants usually settle for less than they should. Often getting a proper settlement would require getting a lawyer.

When my wife finally could not work any more (she had been working with a disability for years), we applied to social security for her disability and were denied. We sought advice about how to appeal the decision and were told we would need a lawyer and then we would certainly get her qualified since it was clear she was disabled but that social security rarely grants disability on the first application. We did not need the money badly enough to go through all that. We would do our best to enjoy the few years she might live instead of spending it fighting bureaucracy.

A friend, white single mom, two kids, minimum wage job went to apply for aid. She got SNAP but the social worker who was not white told her the other programs were not for people like her.

Medical Insurance claims help lines appear to be deliberately staffed with incompetents. I think deliberate incompetence is a business strategy.
